Source-based editorial guide · Not veterinarian reviewed

This original worksheet is an organizational aid, not a validated clinical instrument. Its observation categories draw on the linked Extension care guidance. Print a sheet for a care handoff or keep it alongside the barn calendar.

## Write what you saw

"Left half the usual feed" is more useful than "seems a little off." Note water access, eating, activity and changes in manure or appearance that you observed. If something was not observed, write "not checked" rather than recording it as normal.

## Attach an animal and time

Use a consistent name or identification number, especially in a herd or flock. Write the observation time, not just the date when you enter it later. For a group observation, identify the group and distinguish it from an individual animal record. Keep observations separate from assumptions.

## Close the loop

Add who was contacted and what follow-up was requested. The sheet does not create a treatment plan. Call first for urgent distress and finish the record later. Notes can be entered manually into PawLine, but this prototype does not transfer or store them. Tell the next caregiver where the paper record is kept.

## Your quick-reference checklist

- Animal or group identified.
- Date and time written.
- Facts separated from assumptions.
- Action and care contact recorded.
- Follow-up handed to the next caregiver.

A memory aid only. Adapt to your animals and your veterinarian's care plan.

## How to use this record

1. Identify the animal or group and write the date and time.
2. Describe observed facts. Mark anything not checked.
3. Record contact, action and next follow-up.
4. Keep the sheet for the next caregiver. Call first if urgent.
