Source-based editorial guide · Not veterinarian reviewed

This original blank template is not dosing advice. Do not use it to select a medicine, calculate a dose or decide a food-animal withdrawal period. Ask your veterinarian about instructions and restrictions on milk, eggs or meat.

## Copy without guessing

Keep animal identity, the exact product and the prescribing veterinarian's directions together. If the label and a verbal instruction seem to conflict, ask before administering. This page deliberately provides no suggested medicines, doses or treatment intervals. Resolve uncertainties in the care plan instead of filling gaps yourself.

## The relationship behind the record

A medication record works because a veterinarian who knows your animals wrote the plan behind it. Penn State advises every producer to build that relationship in advance: a veterinarian familiar with your farm and your animals can address health issues more quickly - including when you call about a missed dose or a bad reaction.

## Record the event

A scheduled dose is different from one actually given. Write the actual date, time and caregiver initials after administration. Note any missed dose or concern. Contact the veterinarian about errors or adverse effects rather than quietly rewriting the record to make it appear complete.

## A reminder is not a prescription

The iOS PawLine listing describes medication reminders and dosing history. Those features can organize a veterinarian-directed plan, but a reminder is not permission to administer medicine. This paper template has no reminder service and does not transfer entries to the app. Keep the original directions available to every caregiver.

## Your quick-reference checklist

- Confirm animal and exact product.
- Copy veterinarian instructions without guessing.
- Clarify food-animal restrictions.
- Record actual time and caregiver initials.
- Report concerns and document follow-up.

A memory aid only. Adapt to your animals and your veterinarian's care plan.

## How to use this record

2. Clarify uncertainty before administration.
3. Record actual administration time and caregiver initials.
4. Document concerns and follow-up without changing the original entry.
